I think it occurs when there is an open adjoin and the stormtroopers can slip out of it. I know there's some level where this happens to boba fett and you have to kill him in order to proceed. Unfortunatly he is a phantom so you cannot harm him. Can't remember which level though, I think it was the Dark Tide II
